I think I have it scoped out on how to cur the outer edges of the plate.
But, how do you make the rwo center holes?

Diagonal lines from opposing corners to locate center & use hole saw or fly cutter in drill press. Made one from a piece of bakelite 5 years ago & still in service. Bought a commercial one with removeable center inserts but it's still in the package - no need to change it yet. Rabbetted hole in router table should be slightly smaller than plate, then trim plate edges for a snug fit when it's inserted. Cut the hole & rabbitt too big & there's no going back - except duct tape!
I haven't heard or read the word Bakelite in years. I remember when it was used in radio cases and everything. I'm getting more involved in the design because I want to make the hole so that inserts can be used. Thanks for the reply.
You're right, the piece of bakelite came from a a large piece of electrical equipment that was being scrapped ( I worked 39 years as an Electronics Technologist in Marine Electronics, Avionics, Test Equipment repair/Calibration, & last 10 years in the Engineering section as an Installation Technologist). Tried using a fly cutter to create the rabbitted lip to hold inserts but became a PITA so gave up & just made the hole large enough to accomodate my largest bits (safety police will love that!)
