Make the piece of wood square the size you want across the flats, Set the miter gage to 45 degrees and by trial and error start cutting off the corners until all flat surfaces are the same size. or you could layout (8) 45 degree angles from the center and then determine the size of the flats and cut off the corners.
if you have a sled for your table saw. you can make a 90 degree angle and install it on the sled so that you get all the same size sides. You can draw 45 degree where you want on square piece and then adjust 90 degree angle to where blade cuts where you want. I know clear as mud. but at least make a sled for table saw
