I’d like to fasten a sheet of 1/8″ UHMW plastic to some plywood to make jigs and fixtures. What kind of adhesive would work well?

bob i've roughed up both sides with 60 grit sandpaper, then mixed up a batch of west system epoxy. i used m.d.o. plywood , 3/4 inche but that all depends on what your making and where your using it. i usually do 3'x3' sections and that lasts me awhile and weight it down with a old wash tub 1/4 filled with water dont want to starve either surface from the ep[oxy but enough to keep it nice and flat.......my 2 cents worth....bear
1/8" is thick enough that you could use #2 flat head wood screws, countersunk deep enough to get the screw heads down below the surface of the plastic and still have enough material under the screw heads to hold securely.

Hi, Diver-
I used the UHMW in 1/8" thick sheets for the faces on my table saw fence. I used MDF in 3/4" and applied the UHMW using contact cement. I tried a lot of other stuff and was going to the System 3 next if the contact cement didn't work. I like the contact cement since it's possible to apply it and distribute it evenly. This is important for a fence.
After clamping I drilled 3/8" holes with a forstner bit in a drill press just through the UHMW to allow the drilling for fasteners to the metal fence. Worked well.
