Charles Hummel's book, With Hammer in Hand (University Press of Virginia), has illustrations of shop made screw boxes and taps that the Dominy family of craftsmen made in the 18th century. Also, it shows a plate from Andre Roubo's L'art du menuisier that pretty clearly shows what you need to make a set. The tap made in the Dominy shop is mostly wood, except for the cutter itself, and appears to be for a large vise screw, such as you are wanting to make. Perhaps you can find a copy of Hummel's book in your library.
It seems to me that I saw a description of wooden screw making in one of Roy Underhill's books also. I believe it was also in a back issue of FWW?

Are you looking for a new one? 1-1/2" is the biggest I've seen. Your best bet is probably to make one, like Ray said, or do it on a lathe.
Beall (http://www.bealltool.com) makes a slick one which does 1 1/2".
