Take a wide board, rip one edge at the 45 degree angle, then rip that off the board with a square cut. Don't trap the little piece under the blade.

I'd use shaper (or router) to mill the 45* on both edges of board(s). Then rip off both edges at the table saw. It might be necessary to attach a board to the saw fence fitted tightly against the table surface, so the sharp edge of the strip can't wedge itself as I push it thru. It will help to leave 1/32" of flat at the edge of the strip to help prevent this too.
After ripping, I'd remill both edges of the stock, and repeat till I have enough strips for the job. I'd rejoint to a straight, square edge before milling the 45* after every 2d or 3rd ripping operation.
I'd be alert to the possibility that the strips could kick back. As when ripping anything, I stand to the side. I'd use a push stick dedicated to the 45* end of the stock (if the pieces are fairly short), or pull it thru at the end of the cut from the outfeed side of the saw(if the strips are long).

Just to embellish something that was mentioned, I clamp a short fense of 3/4" melamine to my TS Fense....creating a nice tight fit...and eliminating the possibility of stock getting stuck under the fense. The fense only extends to the saw arbor....helps with any potential kickback. Also, there is a nice video on the home site here for cutting small stock on the TS...
