Alan,
I get most of my curly maple from Groff and Groff and I have been pleased with every piece. The 4/4 is going to be easy, but the 12/4 isn't always in stock; I hounded Sharon at Groff and Groff for at least a year until they got some in.
I once bought some excellent 10/4 curly maple from the link below.

Sight unseen? Absolutely - Irion Lumber Co. I've bought well over $15,000 worth of lumber from them, and not a single board has been dissapointing. Their wood is not on the bargain side, but it's quite inexpensive for what you're getting, particularly in curly maple and/or wide boards.
What is a good price per BF on curly Maple.
>>"What is a good price per BF on curly Maple."So much depends on variables.Your location compared to good sources is one variable. Another is how curly do you want your curly maple to be? If you want some good looking stuff, you could pay anywhere between $7 and $70 per board foot, or even much more.But I just did a clock for my son's wedding present (pics are in the gallery) out of some really stunning stuff. I paid about $8 per board foot. Because it was a clock, it didn't need very much lumber -- but because I wanted it to be really great tiger maple, I bought a lot more than I would need.Because sometimes there is a small amount of truly stunning grain, in a board that is 7 or 8 feet long. In order to get that stunning piece, you must buy the entire board.Which means that I got the (probably) 7 board feet needed for the clock, by buying 5 boards, each of which was 8 feet long.And every board foot cost the same. 5 X 8 = 40 feet of stock, at maybe 7 inches wide....... time $8 per board foot........I won't be laughing at the lies when I'm gone,
And I can't question how or when or why when I'm gone;
I can't live proud enough to die when I'm gone,
So I guess I'll have to do it while I'm here. (Phil Ochs)

That, of course, depends greatly on the quality of the figure. This is why I suggested Irion - the bottom line $ per b.f. may seem high (around $10 a b.f., depending on thickness and width), the wood I've gotten from them has been uniformly spectacular, and would've cost a lot more from another supplier.
One comment about the 150 b.f. minimum - this requirement is actually of benefit to the customer, not so much that Irion doesn't want to mess with small orders. Because this is lumber, not the small scraps that a fair number of internet wood dealers sell, it must be shipped by common carrier (freight). Anything less than about 200 b.f. means the customers paying a very high percentage of the total in shipping.
Also try Collector's Specialty http://cswoods.com/
Good pix online, I have been to their mill, they really care about their products and are generous with rough lumber thickness dimensions
I had some trouble finding suitable 12/4 Cherry stock for some Cabriolet legs from the supplier that I normally use and the secondary supplier didn't have any suitable stock...I understand that Irion has some killer material, but the 150 bd ft minimum (for mail orders) can be a bummer for small orders..
Anyway I had really good luck with Good Hope Hardwoods; http://www.goodhope.com/

Alan,
Berkshire Products is in Sheffield, MA. About a 2 hour ride from you but I buy all my hardwood there.
Look at their web site and give Allen a call.
